Introduction to Integrated Care

Through this module, the student will learn to:

• Appreciate the societal context and the foundational concepts (of populations, consumers, systems and policies) that undergird theory and practice in Integrated Care.
• Understand the constructs for the delivery of Integrated Care, including services and models, capabilities and tools, and resources and financing.
• Contextualise their own programmes within these theories and practices in relation to their own health and social care organisations and to apply insights and processes to their real-world work.
• Critique the pilots and implementations underway with appreciation of the complexities and subtleties of a relatively under-understood domain.

Through points 1 to 4, the student will:

• Plan and design Integrated Care programmes in the health and care delivery services and the community.
• Pilot and scale such programmes for actual patient, family and societal impact.
• Market, enable and activate patients and caregivers to make good use of such programmes.
• Evaluate the efficiency, effectiveness and impact of such programmes, and conduct research to further improve the delivery of health and social care.

What You'll Learn

To meet the challenge of an ageing population, increasing chronic diseases and escalating healthcare costs, healthcare delivery must now be better integrated. Integrated Care is a complex knowledge domain, with macro, meso and micro levels of theory and practice. By introducing broader perspectives to professionals new to Integrated Care, and understanding care models, tools, resourcing, design, principles of piloting, implementation, monitoring and evaluation in Integrated Care, participants will learn how to formulate, implement and evaluate the efficiency, effectiveness and impact of processes and policies to better integrate healthcare delivery.

Minimum Entry Requirement

• Bachelor's degree
• Relevant public health related work experience
• Candidates with other qualifications and experience may be considered on a case by case basis, subject to approval
